How to Implement a HFG Program

Organizations interested in affiliation must complete an HFA Affiliation Application containing four parts:

Part A: General program information such as contact, program start-up and training information. 
Part B:  Specific program demographics, as well as information related to staffing and funding.
Part C: Submit in-depth descriptions illustrating how they will implement the critical elements for effective home visiting programs during an affiliation consultation call. 
Part D: Statement of Commitment to the HFA Critical Elements and requires signatures indicating the program’s commitment to complete the accreditation process and to adhere to the HFA trademark.
